Biopsy kit order form

A biopsy kit contains supplies for two biopsies and includes the following materials:

Labeled bottles containing 10% neutral-buffered formalin
Biopsy request forms ("Request for Tissue Examination")
Postage-paid bubble pack mailers with protective bags

There is no charge for biopsy kits. We will mail more than one kit if requested below. Each time you send in a biopsy, you can check off on the bottom of the "Request for Tissue Examination" form that you want more kits and we will send them to you.
